Protocol terapeutic corespunzător poziţiei nr. 110, cod (J012B): DCI VORICONAZOLUM Indicaţii - sepsis cu scor Carmeli 2 sau 3 - neutropenia febrilă Evaluarea riscului - Carmeli • A. Contactul cu sectorul sanitar: 1) Fără contact 1 2) Contact fără proceduri invazive 2 3) Contacte repetate cu proceduri invazive 3 • B. Tratament AB: 1) Fără AB 1 2) Cu AB în antecedente 2 • C. Caracteristicile pacientului: 1) Tânăr - fără comorbidităţi 1 2) Vârstnic cu comorbidităţi 2 3) Pacient imunodeprimat: 3 - AIDS - BPOC - Cancer - BMT sau 3 Tratament Schema de tratament: Se iniţiază tratamentul cu voriconazol şi după identificarea agentului etiologic se aplică principiul dezescaladării, folosind agentul antifungic cel mai activ Doze: - iv: 6 mg/kgc x 2/zi, în prima zi, apoi 4 mg/kgc x 2/zi în aspergiloză şi infecţii severe cu alte mucegaiuri. În infecţiile severe cu Candida spp: 3 mg/Kgc x 2/zi. - oral (comprimate şi suspensie):  Gr. > 40 Kg: 400 mg, po, x 2/zi în prima zi, apoi 200 mg, po, x 2/zi  Gr. < 40 Kg: 200 mg, po, x 2/zi în prima zi, apoi 100 mg, po, x 2/zi
